To verify identity and process the request, please attach:

– A copy of the Patient’s CPR.
– A copy of the Patient’s Passport.

If the request is submitted by an authorized requester (acting on behalf of the patient), the form must include the authorized requester’s details, and the email must also include:

– A copy of the authorized requester’s CPR.
– A copy of the authorized requester’s Passport.

Proof of authorization to act on behalf of the patient (such as a signed consent/authorization letter or applicable legal documentation).

All identification documents will be used solely for verification purposes and handled in accordance with applicable data protection and confidentiality requirements.

Note: Decisions may be subject to NHRA or other applicable regulatory requirements.
